REQUIREMENTS
Applicants are required to submit the following documents:
Completed Application form
Affidavit to be sworn before a Guyanese Diplomatic or Consular Representative, or before a Magistrate, Justice of the Peace, Commissioner of Oaths and Affidavits or a Notary Public.
Copy of Birth Certificate
Copy of divorce decree (if relevant)
Copy of death certificate of spouse (if relevant)
APPLICATION PROCESS
Applicants are required to ensure that the application form is filled out correctly and submitted with all relevant supporting original documents to the Consular Section of the High Commission in New Delhi.

Collection: Mon-Thurs

All supporting documents must be produced in duplicate (original and photocopy).
Alternatively, applications along with the required documents can be mailed to the Consular Section of the High Commission in New Delhi.
FEES: Rupees 5,500
*Return Courier Fee
In the case of couriered documents, an additional Rupees 600 is required to be paid as a return courier fee
