REQUIREMENTS

Applicants are required to submit the following documents: 

  1. Completed application form (using accurate information-Do not use nicknames). 

  2. Copy of the old Birth/Death/Marriage certificate (if available). 

  3. In case of a name change after the original Birth Certificate has been issued, a document 

verifying the new name (Deed Poll). 

NOTE 

  • Applicants must already be registered. 

  • Applicants seeking to submit an application for minor clerical corrections or amendments to a birth registration are encouraged to visit Minor Corrections - Guyana General Register Office (moha.gov.gy) for further information and submission of these applications.

 

FEES:             

            Birth Certificate -                    Rupess 5,500

            Marriage Certificate -             Rupess 5,500

            Death Certificate -                  Rupess 5,500

 

*Return Courier Fee
 In the case of couriered documents, an additional Rupees 600 is required to be paid as a return courier fee
